Immunization/Physical Examination Requirements
The immunization requirements will be due upon acceptance to the Nursing program, prior to the start of classes.
- Physical - routine: The Physical Examination Form is to be completed by the Examining Provider.
- MMR (Measles/Mumps/Rubella): Full immunity must be demonstrated in one of the following ways:
- Immunized twice with MMR vaccine
- Lab report documenting adequate immune titer
- Chicken Pox (Varicella): Full immunity must be demonstrated in one of the following ways:
- Immunized twice with Chicken Pox vaccine
- Lab report documenting adequate immune titer
- Had Chicken Pox confirmed by physician record
- TDAP - Tetanus/Diphtheria/Pertussis: Documentation of immunization within ten years must be demonstrated.
- Hepatitis B: Full immunity must be demonstrated in either one of two ways:
- Documentation of at least the first of three immunizations upon program admission, and timely documentation of each subsequent dose as they come due.
- Lab report documenting adequate immune titer
- Flu Vaccine: Students are required to receive an annual flu vaccination.
- Tuberculosis: Absence of active Tuberculosis is required and must be documented in either one of two ways:
- PPD (Mantoux) 2-Step TB test.
- QuantiFERON Gold or T-Spot Blood Test.
- COVID-19 Vaccine - recommended: Full series of monovalent vaccine: 2 doses of Pfizer, Moderna or Novavax vaccine or 1 dose of Johnson & Johnson vaccine. One dose of Updated/Bivalent vaccine.
