Immunization/Physical Examination Requirements

The immunization requirements will be due upon acceptance to the Nursing program, prior to the start of classes.

  1. Physical - routine: The Physical Examination Form is to be completed by the Examining Provider.

  2. MMR (Measles/Mumps/Rubella): Full immunity must be demonstrated in one of the following ways:
    1. Immunized twice with MMR vaccine
    2. Lab report documenting adequate immune titer

  3. Chicken Pox (Varicella): Full immunity must be demonstrated in one of the following ways:
    1. Immunized twice with Chicken Pox vaccine
    2. Lab report documenting adequate immune titer
    3. Had Chicken Pox confirmed by physician record

  4. TDAP - Tetanus/Diphtheria/Pertussis: Documentation of immunization within ten years must be demonstrated.

  5. Hepatitis B: Full immunity must be demonstrated in either one of two ways:
    1. Documentation of at least the first of three immunizations upon program admission, and timely documentation of each subsequent dose as they come due.
    2. Lab report documenting adequate immune titer

  6. Flu Vaccine: Students are required to receive an annual flu vaccination.

  7. Tuberculosis: Absence of active Tuberculosis is required and must be documented in either one of two ways:
    1. PPD (Mantoux) 2-Step TB test.
    2. QuantiFERON Gold or T-Spot Blood Test.
    If PPD or blood test is positive, evidence of a chest X-ray is required within the last 3 years

  8. COVID-19 Vaccine - recommended: Full series of monovalent vaccine: 2 doses of Pfizer, Moderna or Novavax vaccine or 1 dose of Johnson & Johnson vaccine. One dose of Updated/Bivalent vaccine.
